Requirements for All Stars:

1.  Original Birth Certificate (no certified copies) *Federal, State or Provincial Registrars Issued
2.  Three different proofs of physical address
3.  Medical Release that has been notarized
4.  All of this information needs to be turned in by Wednesday, June 18th
5.  No exeptions  - if a player does not have this information they will not be eligible to play.
